IMPORTANT NOTES
Even though this form is called an application–all video acts will be in the show
as long as they are school appropriate and
adhere to the time limit.
Each act has a 2.5 minute time limit; please edit your act as needed.
Only ONE form is needed per act.
Please have one parent fill out the form below with all student information,
including the information of the other parents.
It is crucial that we have complete and correct
contact information
for a parent of each performing student.
Each parent will also need to review & sign Variety Show Permission & Acknowledgement Form - Video.
